I certify that the information provided on this application is truthful and accurate. I understand that providing false or misleading information will be the basis for rejection of my application, or, if employment commences, it will result in immediate termination. I also affirm that I have not signed any restrictive document that creates an obligation to any former employer which would prevent me from accepting employment with the company in the position I am seeking.
I authorize Elite Nursing Network to contact former employers and educational organizations regarding my employment and education. I authorize my former employers and educational organizations to freely provide information regarding my previous employment, attendance, and grades. I also authorize the persons designated as references to communicate information regarding my previous employment and education.
Temporary/Contract Employment: If employed as a temporary or contract employee, I understand that I may be an employee of the company and not of any client. If employed, I further understand that my employment is not guaranteed for any specific period of time and may be terminated at any time for any reason. I also understand that a contract will exist between the company and each client to whom I may be assigned, which will require the client to pay a fee to the company if I accept direct employment with that client. I agree to notify the company immediately if I am offered direct employment by a client, or through referral by the client to any subsidiary or affiliated company, either for a permanent, temporary (including assignments through another agency), or consulting position during my assignment or after my assignment has ended.
